MOSAIC
MOSAIC is the new name for Craven’s Community Mental Health & Wellbeing Network, marking an important next step in the development of locally rooted mental health support. Building on three successful years of NHS Transformation funding, the network continues into 2026 with a refreshed identity and a strengthened partnership model.
What makes MOSAIC distinctive is its partnership approach. By working collectively across sectors, the network ensures that people are supported at the right time and in the right way. Shared referral pathways and close collaboration between partners help create a more seamless experience for individuals accessing support, reducing barriers and strengthening continuity of care.
A partnership of 6 local organisations across Craven offering consistent, accessible, and local adult mental health and wellbeing support in the community, managed by local Creative Health charity Pioneer Projects, with support from specialist mental health providers in the Voluntary, Community, and Social Enterprise sector, North Yorkshire Council, Bradford District Care Trust, and the Modality, WACA, and Western Dales Primary Care Trusts.
