MOSAIC

MOSAIC is the new name for Craven’s Community Mental Health & Wellbeing Network, marking an important next step in the development of locally rooted mental health support. Building on three successful years of NHS Transformation funding, the network continues into 2026 with a refreshed identity and a strengthened partnership model.
What makes MOSAIC distinctive is its partnership approach. By working collectively across sectors, the network ensures that people are supported at the right time and in the right way. Shared referral pathways and close collaboration between partners help create a more seamless experience for individuals accessing support, reducing barriers and strengthening continuity of care.

A partnership of 6 local organisations across Craven offering consistent, accessible, and local adult mental health and wellbeing support in the community, managed by local Creative Health charity Pioneer Projects, with support from specialist mental health providers in the Voluntary, Community, and Social Enterprise sector, North Yorkshire Council, Bradford District Care Trust, and the Modality, WACA, and Western Dales Primary Care Trusts.

What’s the offer?

Several open access ‘wellbeing cafes’ with low level wellbeing support tailored to local need and provided by organisations that have trust and reach within their community.

Two ‘quieter’ groups reserved for people not ready to access open provision.

Access to Pioneer Project’s keyworker support, specialist Creative Health activity, and onward referral to partner provision.

‘In-reach’ specialist mental health support provided by statutory and VCS mental health partners.

Additional support including pilot ‘Voice’ groups for People with Lived Experience.

Four of the ‘open access’ groups and wellbeing cafes are just that – open access.

By Referral

For wider support and keyworker engagement Primary, Secondary, Local Authority, and VCS colleagues can refer using the Compass E-hub MARS (multi-agency referral system). If you are not signed up to use Compass then e-mail grace@pioneerprojects.org.uk for a referral form. Those referred receive keyworker support to engage, and access other support, including the ‘quieter groups’
